Playbook: who accepted that risk, and on what evidence?

The situation

The Risk Inbox shows a critical finding: a projected cash-flow shortfall. Finance knows something the system does not — an overdraft facility covers the gap until two large receivables are paid. So they decide to live with it.

Three months later someone asks: *did we know about this?* Without a record, nobody can say what was decided, by whom, or what the numbers looked like at the time.

Step 1 — Record the decision

In Command → Risk Inbox, choose Decide on the finding.

Record a decision: risk accepted, with what you know that the system does not
Record a decision: risk accepted, with what you know that the system does not

Pick Risk accepted, Deferred or Declined, and write the reason. The finding and the recommendation are saved exactly as they are now, so the decision can be reviewed later against what was known at the time. To act instead — expedite, reschedule — use the finding's own actions: the decision is recorded automatically before the action runs.

Step 2 — The Decision Log

Open Command → Decision Log.

Decision Log: when, which finding, the decision, who, and the outcome
Decision Log: when, which finding, the decision, who, and the outcome

Every decision appears with when, the finding, the decision, who made it and the outcome: *not known yet* until the risk either happens or passes. Open one to export its dossier as PDF or JSON — the evidence with a hash check, and what happened after.

Why it matters

A decision is only as good as what was known when it was made. Because the evidence is frozen, you can judge the decision fairly — and see over time whether accepted risks tend to happen.

What you have at the end

  • Every risk decision on record, with who and why.
  • The evidence exactly as it was.
  • The outcome, tracked.
